We're thinking of adding features that other OSQA-instance owners might not need. Of course, we still want to get the latest updates from the trunk.

What kind of features are you talking about? It might be worth floating them on the mailing list. You'd be surprised, it might be that a handful of other OSQA users want those features too.

If you use OSQA modules to build your extensions, then you'll be able to do a lot without interfering with the core of OSQA at all. Use caution if you start changing OSQA core code, and be aware that you'll be on your own when it comes to support.

good question. A extension you will really need, when you try to make a production. e.g.:

  1. high redefinition front-end view
  2. some feature your client need, but there is not in the current version etc.

take look magento shop system, there are some many extension to use or you can develope your own extension.
